Requirements

What to Bring to Your Fingerprint Appointment

Form 3, provided to you by the CBC (unsigned).
A Valid Government Issued Photo ID. Acceptable forms of government-issued ID include: driver’s license, state issued Identification, passports, tribal IDs, and military IDs. You are only required to bring one form of ID.

What to Expect at Your Fingerprint Appointment

Our officer will check your ID and witness you signing form 3.
Our officer will complete and sign the bottom of Form 3.
After your fingerprints have been taken we will will ensure your FD-258 card is complete and signed by you and our official.

What to Do After Your Fingerprint Appointment to Complete the Process

Mail your fingerprint card and forms 1,2 and 3 to the CBCP office.
